Patient Management: HPMS allows you to enter referral information and track accordingly. Referral can be converted into patients to store all patient demographic information related to patient care. This includes assessments, a history of the patient’s care planning, medication administration records, hospice resources, insurances, and durable medical equipment. Next-of-Kin and primary caregiver captured is utilized for CAHPS Survey reporting.
Patient Careplans: Automates the process of identifying patient problems and producing a careplan that may be modified as treatment progresses. The careplan includes care needs, goals, and interventions. Careplan review is easily accomplished during interdisciplinary team meetings providing for medical update and approval using physician orders.
Bereavement: The Bereavement component is designed to assist hospices with follow-up care to family members. The primary care giver and next of kin data is extracted from the patient file into the Bereavement file to reduce manual data entry. Non-patient family community entries can also be created.
Billing: HPMS automates the billing process. Charges are calculated based on the patient’s level of care or the services provided during the billing cycle. HPMS formats the data in accordance with NUBC specifications.
EMC (Electronic Medical Billing): HPMS formats billing records to the specifications of your financial intermediary to ensure clean claim submission. No on-line manual data entry is required for the EMC transmission. Electronic Remittance Advice Importing is also available which greatly improves the processing efficiency and reduces errors in the application of payments.
Accounts Receivable: The Accounts Receivable component allows you to track incoming reimbursements from insurance companies and patients, as well as make adjustments to accounts. Posting and accounts receivable aging reports are available to aid in management decision making.
Mailing Lists and Fundraising Management: The mail component enables you to target groups within your community to receive hospice mailings, track donations and pledges, and produce automated thank you and acknowledgments.
